CONSERVATIVE TREATMENT OPTIONS FOR PATHOLOGICAL PLACENTAL ATTACHMENT IN LITHUANIA: THE ROLE OF TELEMEDICINE
Abstract
Placenta accreta spectrum (PAS) management is a complex problem that requires extensive health resources and skilled multidisciplinary teams. Until now, in Lithuania, there has been no consensus on the best care for PAS cases. We report the process of incorporating uterine-sparing surgery in Hospital of Lithuanian University of Health Sciences Kauno klinikos through international collaboration facilitated by telemedicine with a hospital located on another continent with the successful execution of four PAS cases in six months. Two cases of placenta increta and two cases of placenta accreta suspected antenatally on ultrasound were referred to a center of excellence for PAS where the standard treatment was hysterectomy. The interaction between a hospital already managing one-step conservative surgery and the local group occurred in several phases. Uterine-sparing surgery was successful in all cases and had beneficial clinical results. Telemedicine can help incorporate new surgical techniques for PAS patients in hospitals that previously used hysterectomy as the only management alternative.
